Insurance

As a courtesy to you, your primary and secondary insurance companies will be billed.  Prior to scheduling your visit at Bay Eye Clinic, review your insurance policy(s) to determine vision and/or medical benefits.  We are required to bill your insurance based on the reason for your visit. You need to provide the complete names and addresses of your insurance  companies as well as complete group and membership numbers.

Please remember we have treated you as our patient and not your insurance company:  the basic responsibility for the payment is yours.

Many insurance policies now require a referral from your primary care physician (PCP) before you can see a specialist.  Please check your policy and contact your PCP to obtain that referral prior to coming for your appointment.  If a referral is not in place when you come for your appointment, it will be necessary to reschedule your appointment.
Various insurance companies also have a co-payment that you need to pay prior to seeing the doctor.  If you are not sure if your insurance requires a referral or co-payment, please check your policy.

All physicians at Bay Eye Clinic are participating Medicare physicians. This means that they accept what Medicare approves.  You are responsible for the deductible as well as the 20% co-insurance.  After your deductible is met, Medicare will pay 80% of the allowed amount.  You are responsible for the 20% that they do not pay.
